District court properly applied abuse of discretion standard to review plan's decision to deny benefits in bowl transplant case; while plan may have had a conflict of interest because it was partially self-funded, plaintiffs could not establish the other factors which would call into play a less deferential standard of review; under abuse of discretion standard, the decision to deny benefits was supported by substantial evidence.
