District court did not err in applying sliding scale abuse of discretion standard based on its finding that there were procedural irregularities in the plan's decision to terminate benefits; decision to terminate benefits was unreasonable, given the plan trustees' interpretation of the policy in question was clearly contrary to the language of the plan, and that they failed to comply with ERISA's procedural requirements.
