claims under the Missouri Prompt Payment Act; district court did not err
in dismissing plaintiff's RICO claim; district court did not err in
declining to enter an injunction setting forth the manner in which United
was to handle future claims as plaintiff had an adequate state law and
ERISA remedies for future violations.
