Civil case - Social Security. The ALJ did not err in deciding to give the
opinion of one of claimant's treating physicians less than controlling
weight as it was entitled to relatively little evidentiary value because
it was rendered on a check-box and fill-in-the-blanks form and gave no
elaboration; further other objective evidence in the case contradicted the
doctor's evaluation, as did the evidence of claimant's daily activities.
